Default Went to my first Informational Seminar tonight and I have a few questions!!!

I went to my first Informational Seminar tonight with Dr Procters group and even though most of the information they talked about I already knew since I've done so much reseach on the internet but it was nice to be able to actually ask questions and meet the Dr's face to face. Anyway, we all recieved a packet to be filled out for consideration for the WLS. I was filling out what I could of the packet tonight and when I got to the weight loss section I started to panic. They need you to list your weights from when you were born all the way up to now, well heck I don't even know how much I weight when I was born and I'm hoping my mom remembers but I also have no idea how much I weighed when I was 12?? I know I was overweight but not MO at 12, I'm starting to freak because I don't remember how much I weighed at 18-24.
I know that I've been MO for at least the last 2 years and obese for at least the last 5 years. I have tried numerous diets all of which lasted for a few months before I would give up and gain most of the weight back. But I never really kept track of my diets and if I did I don't know where those journals are now!! Now they want me to list every diet I've ever been on and how much weight I lost and how much I regained.

How many people actually remember all of this stuff? Did you make a good portion of it up?
Except for going to the OB when I was pregnant I never really go to the Dr, maybe 1x a year for the past few years so basically I can count how many times I've been to the Dr on 1 hand.

For me, weight has been an issue all my life...I remember the numbers. Try looking back at old pictures to give you an idea or old clothes you wore then to help figuring out what size you might have been. Does your mother have a baby book for you - she may have listed it there. My doctor really likes to use the age 18 as a guideline, then he talks about my weight from there to now. Did you every go to a weight loss program where they tracked your weight? What about a doctor weighing you at some point?

I know all that paperwork can leave you feeling overwhelmed ! I did remember key moments in my life when I peeked out from behind the curtains of denial. I used those numbers and gave my best guestimate. You can kind of wing it, not lie, just give your best answers. Good luck with all of it. The diet page was more overwhelming for me. It was hard at first to remember the dates, what did surprise me though was when I began listing the diets, a fair amount came back to me and I was able to put the weight loss/maintained/gained. You'll do fine. Don't let it freak you out too much.

Thanks everyone!

I called my mom and she could recall a few things from my younger years that I couldn't seem to remember, like going to a nutritionist when I was 15 so she helped me put a few years together. I can remember estimates on when I was on all of the other diets but exact weightloss and exact weightgain is a little harder to remember. I basically put "about 20lbs lost" and then "about 30lbs regained" I'm hoping that's ok with them.
