Mine doesnt, but the Dr group I belong too does (I have Health Net). I have a awesome DR so I wouldnt switch for anything either! Its a HMO. But on the bright side I am learning a lot in the class, and we have really nice Dieticans who run it. SO all in all the 6m. class sounds awful, but now that I am 1/2 way through it isn't so bad. I know there is a insurance out there that does require 5 YEARS of documented weight from your DR. That seems extreme. I don't remember the name of the company tho....I could find out for you on the 17, if you want to wait that long..........LOL
Good Luck~

Me too......I did have to prove "past attempts" and a 5 yr history of obesity. But those I had no problem with. Good luck......also, I don't even know how much mine was, being that I never even got a bill. I basically paid co-pays along the way and a program fee to the program that was not medical. Love my BC/BS!! They also do things like reimburse $150 a year of a health club or fitness plan.

I had Health Net - my medical group was Sharp-rees Stealy. Neither req the 6 months.
I had Kaiser previously and they do req the proof but it doesnt have to be from a "doctor" theyll take Weight watchers or jenny craig etc. THEN after they get that proof they make you go through THEIR 6 months. Kaiser takes forrreeeeevvveeerrrrr...

I have BC/BC PPO Federal employee and they don't require the 6 month diet or 5 year history (Thank goodness!) all they care about is being 100lbs over weight - they don't even care about co-morbidities - go figure.

I have United Health Care EPO Choice Plus and they did not require a 6 month diet and neither did my doctor who was awesome (Dr. George Mueller) but he did want to hear of my past attempts...

I got BC/BS of south Carolina, they do not require any diet documentation as a criteria. My secondary insurance is Capital Blue Cross od Southern PA., they require 3 months.

I have Anthem BC/BS and they didn't require anything other than listing the diets that I have tried in the past, although I did have documented proof of dieticians and weight loss attempts from the doctor. Hell, I had a lifetime of documented obesity, so that was a breeze. Whatever they require, it's so worth it in the long run to have this surgery. I'd dance naked in the moonlight if they required it.
