I didn't either and I have Medi-Cal (Medic-Aid in other states). I had to write down all my dieting attempts...and I do mean ALL of them: cabbage soup, Atkins, Slim Fast, Dexatrim, Phen-Fen, Meridia, Xenical, you name it.

I have some form of UHC and all the required was BMI of 40. Heck the surgeon required me to do more than the insurance company did.

I have Aetna & I just found out today that they require a 6 month diet, exercise & behavioral modification therapy. Their guidelines I found online said a 6 mos physicain-supervised diet & exercise program OR a multidisciplinary surgical prep & I did the 3 mos. So I found out today that I have to do the 6 mos. I feel like this is not fair & I am so depressed over it. I want & need this surgery bad!

I had Blue Shield HMO and had to submit documentation. I had 3years of proof and they denied me the first round. I think they wanted to see if I would appeal.

Default Yet another BC customer.....

I have Capital Blue Cross PPO Premium, and I didn't need the 6 month supervised diet. However, my surgeon required that I document my weight loss attempts, and that was provided to Cap BC along with all of the paperwork needed for the approval.

Hi - BC/BS in what state...I am being told that it might get rejected becaused I did not have the 6 month doctor suppervised diet. However, at the doctors request tried Xenecal for 3 months...did not work.

You have a great story...congrats.
