Default Dr's office calls today....

and tells me that my PCP only wrote in the notes once during the entire 6 (actually 7 ) months of dr visits about my reduced calorie diet and nothing about my exercise plan which was really only increased walking and using my hand weights and resistance bands.
Has anyone else had this issue and how was it resolved? I am thinking I will have to contact my Dr and have her go back over the notes and write it in fax it again. Which will most likely make that girl who had to fax my file last week roll her snooty little nose up again....

Write the letter FOR him - give to him as a suggestion that this is more what the insurance needs. I hope you kept notes and know WHAT you ate, WHEn you exercised, etc.

Unfortunately, I did not keep a food journal. I always had good intentions of starting one but never did. Going by the letter I received from BCBS of what they required nothing was mentioned about documented food diary. I just assumed that the Dr was adding in that info into my files as it was the diet plan she put me on. As far as exercise goes she only told me to walk as often as possible and continue using my hand weights and resistance band for exercise. That's it.
