i was denied at first because i think it was an oversight on thier part. They claim i didnt do all the requirements but as you see i did. Girl i live WAY out on 107ave/Indian School so you dont want my PCP cause hes all teh way out here but my surgeon was Steven Simon, MD he's now out of Tempe St Lukes his # is 6022640910 You can call them and see if they can direct you in a PCP or what the steps are.

thats just the 1st criteria that you be medically approved BUT you will need 6months (at least ) of PHYSICIAN supervised diet. not WW or any of those, but your PCP will have to send you to a dietician and you go once a month, #2 you need to do monthly weigh ins with your PCP so he can keep that in your record. #6 months of physical therapy set up by your PCP. And you need all the testing sleep apnea, etc...

It was supposed to only take 6 months but because they need those appt. back to back every month it wound up taking 1 1/2 years to get approved (and i was denied after i did all of that then made an appeal and was approved) I would tell you that you need to get in really good with your PCP's referral person because she knows exactly what you need to do and if their wise she will stay on top of that. Its tedious but now that i'm 20 months out i'm SO glad I waited and stayed on top of things. Let me know if i can help you (i'm in the way west valley lol where are you located?)
