No, not all require a 5 year history of attempted weight loss. Mine required a 6 month medically supervised weight loss program.
I would call again and see if you can talk to someone who will give you a straight answer what their criteria is. If they have a website you may be able to find it there too. I know that's where I found the info for my insurance. I just did a search on the site for weight loss surgery or bariatric surgery.
Your pcp may also have an idea if he/she has referred patients for lapband before. Also most surgeon's offices have someone who deals with the insurance end of all of this.
