Yup, "WeightLoss Surgery for Dummies" is a great book. You also might want to read under the Personal Stories thread. There you'll see some our own experieces from the good to the bad.
As far as the insurance goes, call them directly; the web site usually only gives generalities. When I called mine, I found out there are some differences between some group plans. The customer service rep can look up your specific one and let you know what they require to cover it. Mine required a PCP referral and 3 co-morbidities, and a documented 6 month weight loss attempt. (and by documented, they mean documented by a doctor!) I met all these criteria, and got my approval 3 business days after the surgeon submitted his paperwork.
