Does anyone know how many years of medical history does bc of ca require?

I would guess that it would very from Dr. To Dr. and fro insurance company to insurance company. So the best think to do is ask your PCP.

For BCBS of MN it is a 3 year history. The best thing to do is either call your surgeon's office and ask or call your insurance company about it. They will give you tons of info that will make your head spin, well at least it did mine.

Thank you all for the info.I called the insurance company today to find, but when the rep got on the phone he told me I was approved. So I forgot to ask after hearing approved.
