Call your insurance company. You need to know if they cover WLS at all, and if so, what their requirements are so you know if you even qualify. If you do qualify, you need to know what they require you to do. Like, do you have to do a six month diet? Do you need a referral from you PCP to see a surgeon, or can you just make an appointment with a surgeon on your own?

I started with my insurance to make sure it was coverd and find out what they required. Then I went to my PCM and got the referral for the surgeon.
