Ok, I went to see the surgeon and we went over the criteria per my insurance. I have anthem bc ppo. Now it asked to show failed attempts of diet but it didn't have a specific. So the nurse had me to come in each month because she wasn't sure if I have to show a 6 month diet because it didn't say. Has anyone with the same insurance experience this? Also I have to show proof of weight history again it didn't say 1yr, 2yrs or 5 so I don't know what they are asking for. It was friday when I went so I couldn't call the insurance company.

I'm not going to be of much help, but the best thing is to call your insurance and get specific requirements. My PCP had been seeing me for two years, and documented each time that I wasn't being successful in losing. My Insurance only required two years of being overweight, although I had been for around 8 years. It shouldn't be so complicated, but it is. Good luck on your journey.
