UHC covers it, BUT... some employers choose to exclude it from their plans. So to find out if it is covered on YOUR plan, you'll need to call UHC and ask. While you're on the phone with them, if they do cover it, ask them to send you the criteria in writing. Because if it is covered, it will only be covered under circumstances, and you'll have to do certain things in order to get it covered.
