Default Exactly what is needed?

What is needed for the surgeon's office to submit to the insurance company? I ask because they just got all my PCP noted for the last two years but did not receive my psych eval yet.

Anyone know if they need the psych eval to submit?

Yes, they need it all. Whatever tests/evals your surgeon made you go to, they need the "whole package" to show to the insurance.

usually the surgeon has the whole package and a checklist of things and tests that are needed both for his requirements and the insurance.

Thanks for answering. I have everything almost in, but the psych doctor is taking his good old time. It has been over three weeks I have asked for it and calling each week. He is really holding me up. The surgeons office said they don't need the mam and pap test to be approved. That I have scheduled this month. I am just frustrated.

The MORE information that you can give an insurance company.....the better.

But then it should say in your policy, under bariatric part.....what they require. Speaking from experience...(I fought Aetna for over a year for my surgery)....go by the policy and submit everything on it.

I am glad everything worked out for you. I have my hopes up that I will hear something soon after submission. I have started trying protein mixes and I know people say your taste changes after surgery. My doctor told me not to even gain a pound between appointments so I am trying to keep my weight down.

I am trying to get the psych doc to fax over the info, so far nothing. I know he approves of me getting it, we talked about it and he said he was for it. I just think he hates to do the paperwork..lol

Keeping my fingers crossed he sends it in today... I hope anyway.

I think because it is easier on you during surgery. I really don't want to gain weight. I have lost 121 lbs on my own. It took me five years. I still have about a 100 to go.

I was told it didn't matter if I gained or lost.
