varies from insurance company to insurance company... the first time I met with the people at my surgeon's office, they gave me a list of things I needed to do before they submitted my file to insurance, based on the company I was with (aetna open choice).

I think I needed:

-3 months supervised diet
-psych eval
-2 year weight history (I submitted a physical from 1999, my records from when I was hospitalized in 2005, and a physical from 2008 which I guess was good enough)
-letter of support from PCP
-cardiac clearance (this was actually after insurance approval... the anesthesiologists at the hospital requested it and I guess it's always good to find out that I don't have any heart problems)

the whole process took about 3 months and I got insurance approval like 2 days after my surgeon's office sent it out.

I have heard that if you gain - you don't get approved.

I believe that. My doctor seems very strict about that. And, everything else. So I wouldn't doubt it. I have been watching what I eat and drink two protein drinks a day. I have to go to another psych, mine would not write a letter for me. Oh well..... I have an appointment in two weeks.

Trying hard to get things in order.

i think they just want to show that you've made some effort...
i have stayed the same and i was told that would be OK.
