Default Need Help Finding a Post

A day or two ago I was reading a post that had details of exactly how a Dr.
should document your 6 month diet for insurance purposes. Unfortunately,
now I can't find it again. Since my pcp has had very limited experience
with wls, I'm concerned that he will not cover all the bases with his report.
Does anyone remember this post and if so can help me find it again?

I have bCBS IL and the way my paperwork reads is date, PCD name
patient name. DOB
height weight. BP pulse, reason for exam -patient desires medically supervised weight loss. subjective
exercise type & amounts per day/week
diet:
pharmacological- if not utilizing, please document reason why not.
behavior modification: Dietician, psychologist, support groups, internet/online.
comments, and objective findings.
recommendations: Encouraged continued dietary modifications as above. Discussed with patient their progress toward his/her goals, including continued exercise and behavior modification efforts. Recommeded increased protein and water intake, increased exercise, and decreased carbohydrate intake as preparation for the post-weight loss surgery lifestyle.

Physician signature: and date.

that is exactly how mine is typed up on a paper.
