Can you believe it?? I forgot to send in my 2 yr weight loss history. I called the insurance today and you could have knocked me over with a feather! So now I have to call my pcp and have her fax the weight history (I have been seeing her since 2006)to the surgeon. Does she need to send a copy of my weight from one visit for each year? Also, how do I need to send this, as an appeal? Any help would be really appreciated.

at first mine was denied b/c my drs letter didnt state that my dr was aware i was going to lucielle roberts womens gym and getting monthly weigh ins.
you arent really denied its just you are denied until they get the proof of what they need.
once i sent in what they needed i was approved. yours should be the same.
