I made copies too for my PCP. I actually placed on a word document so I can have the PCP place in my file chart. I made extra copies so he can one readily available when I come in. This covers everything I need all on one sheet. THANKS SOOOOO MUCH this is very helpful for me!!!!!!

I told my surgeon about it. I think it would be good to have doctors on here that can answer question if they have a little spare time to kill. There may be some on here already. I don't know yet (Blush) He seemed pleased that I found the place and was so well prepared

Not yet but that probably a good idea. I'm so glad I don't have to come up with the 5 year diet history or Id be screwed for sure. I was never a Dr. person so Iv really only had a PCP for almost a year. lol Figured I'm getting old and feeble I should break down and get one. lol. Never dreamed Id ever have to Document my struggle over the years. Most of my diets have been on my own. Id done them all too. lol

are you guys on cigna too? because i think that is the only one that requires you to have the diet.
is your pcp doing it or is your surgeon doing it?
i have asked for help on a thread but no one has bothered replying.

i started by diet and just finished my first month a few days ago. held steady - no weight loss. pretty par for the course for me.
