what kind of insurance do you have?
I am doing a study on what kind of insurance gastric bypass patients have.
It would be a great help if you could repond with
1. Type of Insurance (Blue cross, healthnet...etc.)
2. HMO. PPO. Self Pay Insurance, Cash, Medi???
3. Your doctors name and state.
