I have not had my surgery yet, but in talking to the reps Anthem Ble Cross HMO 100% covers it 100% except for my $10 copay for doctors visits and $500 copay for the hospital stay.

I was a self pay w/o insurance through my employer being affordable (and the exclusion they have anyways)
