Default Where to find criteria info on ins. website??

Anyone happen to know where I would find the exact criteria for WSL on United Health Cares Website? I have looked and know they do cover it, but cannot find the criteria. Thanks to anyone that can help!

I have gotten a pretty vague description of what is needed from the insurance company over the phone, but I haven't been able to find it on the website. I would just like to see in writing what they need so I know I am definately not leaving anything out. The insurance company didn't say I would need a letter from my PCP , but a few others on here told me I definately will, and UHC also didn't say anything about a dr supervised diet, and it seems everyone has to do that. I'm just really worried I won't send enough info and get denied I guess.

Cool I think I know...

Hi, I am a United Health Care patient. Pick out a gastric bypass surgeon and he/she can give you a print out of what they (UHC) requires. Mine: pych evaluation, 6 months supervised diet, at least 100 lbs overweight, 40+ BMI, sleep epna test, heart and lungs test, dietiation, and would help with medical problems, dietiabets (sp), blood pressure etc. Good Luck

I have United Health Care and I got approved in 4 weeks. A lot of it has to do with your employer medical plan. Your employer decides what they want to cover on there medical plan. You have to have co-morbidities along with being over height. For instance, I had joint pain when I walked. Other then asthma I had no other co-morbidities and was approved. There are lists of them on the internet.

Lori,
Where did you find the list you were talking about? My doctor should have my test results for my liver, blood sugar and kidneys back today, so i don't know what those results are yet. I know I have IBS and sleep apnea and asthma though.

Here is a list of Comorbidities.

Obesity is Associated with Significant Comorbidities
Osteoarthritis.
Hypertension.
GERD.
Urinary stress.
Incontinence.
Gallbladder disease.
Depression.
Diabetes.
Hyperinsulinemia.
Asthma.
Sleep apnea.
Congestive heart failure.
Anemia.
Neoplasia.
Dyslipidemia.

Comorbid Conditions
Almost 80 percent of obese adults have one of these conditions:

Diabetes.
Dyslipidemia.
Coronary-artery disease, hypertension.
Gallbladder disease.
Osteoarthritis.

Almost 40 percent have two or more of the above conditions.

Ok, so probably a stupid question now, but I had to have my gallbladder out a couple years ago...would that be a comorbidity??? I was very over weight at that time too. But does it still count now???
