does anyone have one?

can anybody get their letter from their pcp of medical nessesity that is sent to the insurence company prefferably if it was approved or can any body get one.I want to make it as easy as possible on my pcp so she cant deny me i want to hand it to her and just tell her sign right here hopefully someone can help please
