Sounds similar to my situation. My doctor ended up "doctoring" his notes to include weigh-ins and a 6 month supervised diet. He wasn't straying too far from reality, as those things were being done, but not to the standards the insurance company wanted. His actions got the ball rolling immediately. Worst part is, my surgery ended up costing them less than what they paid the previous three years for all the co-morbidities.

just wanted to say good luck it took me a year and half but you know what i'm 4 months post op so it really does go by quickly
