Hello, I had straight medical, under community health group. I did not have to do anything as requirements for my surgery. Yes I had to do a psych evual. and internal med clearence. My pcp put the referral in and I went to orientation. Then Dr. Tanaka's office put in the request for the surgery. My surgery was approved in less then a week. I think all offices are differant on billing. But why they would do the surgery before they know if approved by insurance is beyond me. Good luck.
