Default Doctor note?

Hey I have an appointment with a doctor to get a note for Gastric Bypass. I was wondering will he really give me a note? Or will he have to give the note to the insurance I'm sending it to? What will this consult be like? Last time I saw him he mentioned Gastric bypass would be something I should look into and so that is why I'm going back to him to get a note. I'm not sure what happens really though?

My PCP wrote a letter supporting my request for gastric banding. I did not get to read it, but it included a summary of my diet attempts, the medications he had me try and a history of my weight. Insurance companies will want to know that you've been morbidly obese for more than 5 years and that you've made some serious attempts to lose weight.

More than likely he will be sending a letter of recommendation to your insurance company but you might ask to have acopy sent to you. It's funny-not how the mail and faxes just seem to 'lose' paperwork that will effect how long we have to wait for the approval. I had a doctor in the sleep disorder department e-mail the person who schedules surgery This scheduler was on materity leave and by the time the results were faxed and recieved, it was an extra two weeks! Oh and the most irritating part is they are at opposite ends of the same building!! If I had a copy of the results, I could have walked them over within 10 minutes!

My doc wrote my letter, and offered to fax it to my surgeons office, so that it could be sent in to the insurance company with my packet. I however asked to hand carry it, as I know how things have a way of getting lost in transit.

My doctor is writing me a letter as well. I have to go pick it up from her office when it's ready.
