OMG! They have vanished. One of them with absolutely no trace.

Smile Just document it

There were several Doctors and specialists that I saw for weight loss prior to deciding to have the surgery done.. however a few of them no longer exist either. I still put them down anyway.. I couldn't get my records from Jenny Craig, Weight Watchers, or Nutrisystems because their records don't go back that far or so they said.. I didn't need them I guess because I was still approved. Just document any and all things you have tried including but not limited to over the counter remedies and prescription medications. You'll do great.. good luck. Also next time could you please sign your name so we know who you are sorry I'm into personalization these days..

Smile Vanishing MD's

I agree with Christina, list everything you know about them and let the insurance carrier try to get the records.... the insurance carrier can only hold onto the surgical for 30 days... so they will get the records or not even bother going through all of the leg work and take your word for it. Good luck girl!

I had a weight loss MD for a year while on phentermine. When I went to get copies of records, lo and behold, he has died of a heart attack! He was a solo practicioner and who know where the records go when a doctor dies.

Just list everything. The more thorough the better. On my list, I had it down to specific dates for the past 10 years. I had to put it all in 8 font to get it to fit on a single page! Every fad diet, even if only for a few weeks. It all counts.

Default Peckkale,

I went to at lease three Doc-In-The-Box clinics also for Phentermine when Fen-phen was determined to cause heart valve problems in some patients. Then I started going to Mexico for it, but that's another story. So, what people have said before me; just write down dates of attempts and method used. I wish you the best the luck.
