Tess I'm not sure if your Humana plan has the same clause as mine but it might be something worth asking your patient advocate about at your surgeon's office. I know mine does because I requested a copy of the bariatric coverage from the insurance company. My coverage was explained to me by the bariatric nurse case manager at the insurance company. The insurance company also recommended that I get the lap RNY since my BMI is greater than 50. My patient advocate/surgeon said that's not an absolute because you have to look at the individual and not just a number.
