I think I saw on another thread that you are a 39.5 BMI. With those comorbidities I'd say there's a good chance you may still be approved, just keep massaging the system and it'll work out for you. Make sure you have all of your documentation lined up (any additional info would be good probably, too).
I have Kaiser so the system is a little different, I don't know if insurance companies want to see picturs and a letter from yourself or not, but I'd say it couldn't hurt to show you are ready for surgery. (I had a binder organized with pictures, food/photo/written journals, the research I have done, and the other diets I have tried. The doctor didn't need to look at it, but if he wanted something more I can't see that would hurt.
