depends on your doctor and your insurance.
my insurance made me go through 3 months of seeing a nutritionist, which was the biggest hold-up in my case... once I was done jumping through all the hoops (nutritionist, letter of support from pcp, psych evaluation, etc, etc) and my surgeon's office sent my stuff off to my insurance company, I had approval within a day or two and my doc's first open date way 2 months after that (though I elected to push it back a month and a half further back since it worked better for me schedule-wise)
