Hi there, Shetaz.. welcome to TT! the answer to #1 is to ask your insurance company as was suggested already.. they are your best source of this information.

The answer to #2 is.. probably not, however it might mean you will need to do the six month thing first. I didn't have to do the six month pre-op diet due to the fact that I had BCBS of MA and they allowed it to be waived if I met certain criteria, which I did, plus the fact that I had been with my PCP for 15 years and she had documented all my past attempts at weight loss.

I know some here have written detailed letters to their insurance company's, explaining all their past attempts and it seems has helped some get approval.
