I had Cigna HMO which was going to be change to Open Access 80/20 on jan1st, 2008.....So i pushed hard and had GBS on Dec 28th, 2007 under the HMO. They paid 100% of the actual surgery minus $250 hospital copay...other cost break-downs.


PCP Referral - $25.00
NuWeigh Initial Visit - $40.00
Psych - $100.00 <<<< Used the clinic psych instead of insurance.
Nutritionist - $50.00 <<<< Used the clinic nutritionist instead of insurance
Blood Work - $20.00
Gallbladder Ultrasound - $40.00
Sleep Study - $40.00
2-D Echocardiogram - $40.00
PCP Clearance - $25.00
Hospital Co-Pay - $250.00
