If anyone has a good predetermination letter they could send me I would really appreciate it. I go to the Dr on Wed and would like to get her to fill it out so I can find out if I am covered or just wasting my time. I was told by my insurance wls was excluded "
Unless Medically Necessary" but told by my HR dept that the CPT codes I send her showed WLS is not covered but I should have my dr to submit a pretermination letter. So if anyone has one they could email me my email is:
