I don't plan on putting my name on anything. I'm hoping the insurance company won't have to use it either. I'm thinking that I'm going to have to "prove" that I received the information, via my email account.

5/09/08 ~ Denied!!!! WTF!
5/29/08 ~ Appeal Denied! (Aetna shows NO paperwork from my doctor, so I have only been denied 1 time)
10/10/08 ~ Sent Appeal off to Aetna (waiting again).......Aetna got the paperwork 10/14/08
