Most, if not all, health insurance companies will deny individual policies for health issues far less significant than gastric bypass surgery. If at all possible you probably will do best continuing your policy via COBRA.
I have health insurance through the California Major Risk Medical Insurance program which offers insurance to people unable to get individual policies.
