I would go into it with the mentality of no carbs and sugars ever again. When you have lost a significant amount of weight you can treat yourself, but you may or may not dump, and you need to be aware of your trigger foods that may make you lapse into your bad habits. For me it is bread, pasta, potato, so I stay away totally.

I can tollerate splenda without a problem. Some "Sugar Free" foods that are loaded with sugar alcohols still give many people a problem.

I don't do protein supplements so I can't help you there. Each doctor has their own preference. Mine wanted you to reestablish good eating patterns and only use supplements if you can not reach your protein intake goals.
Vitamins are maybe $20/month in total once you add them all up
